Personal Definition of Design Fault: An artifact or system which does not carry out its function or does not work ergonomically after a short period of time. 
Design Fault at my home: Simply, our dishwasher does not wash dishes, it solidifies the dirt/mess that is already on it. 

They look cleanish because we wash the dishes and then

put them on the dishwasher because my brother and I are

too lazy to dry them.
